Great Staff, Clean Facility
I have been very happy with the care my father receives here. Whenever I visit, the staff always accommodates me. The community is also clean and welcoming. Its comforating to see my dad well-cared for, especially because we had a hard time deciding which facility to choose. Glad we chose Meadow Falls.

I was really impressed by how well the residents are cared for. During my visit, I had the chance to witness one of the residents' activities, and it was wonderful to see how much fun everyone was having. It was clear that the team puts a lot of thought into keeping the residents active, social, and involved.

My brother has been here for years now. He has been surrounded by great people who truly care. The community is clean, inviting, and warm. The staff is friendly, and they have kept him engaged since day 1. Highly recommended.

Brookdale was like home. They had good food and a mostly caring staff but not enough of them. There was limited space outdoors. The maintenance guy was amazing. We had problems with medication which is a critical problem.

This facility has been a blessing for our loved one. It's nice to know that they can stay in the facility even on Medicaid. We looked at so many facilities that took waiver and this one was the nicest in the area by far. The rooms are even updated!

I recently visited my mom in the facility. She looked comfortable, well cared for, and in good spirits, which means everything to me. She shared how much she enjoys the activities and the friendships she's been building, and could really see that reflected during my visit.

The administrative staff has been very helpful and, for the most part, very friendly. One area that could use improvement is staffing—having more consistent full-time nurses would be beneficial. They sometimes rely on agency nurses, and because of that medications can occasionally be delayed or mixed up. As for the food, it could definitely use some improvement. That seems to be a common complaint in many places, and most of the residents don’t seem very thrilled with the meals they receive. They do appear to provide a good number of social activities. During the summer, residents are taken to the store once or twice a month, which gives them an opportunity to get out and about. We haven’t had the chance to experience that yet, but it sounds like a nice option for residents.

The staff at this building are very caring. Facility is clean. The kitchen serves a wide variety of comfort foods. It's nice that we are also able to eat a meal with my mom when we come to visit. Location is close to many amenities.

I was overall impressed with cleanliness of facility and lack of sterile feel or smell.. more like a hotel or nice resort. Staff has been incredibly friendly and willing to work with our needs.. only gave a 4 due to billing issues I have had and cost issues.

The facility has a home type of feel to it. Two beautiful indoor cats roam around the building named [name removed] and [name removed]. Most of the staff are friendly, welcoming and some of the staff will help more than you think. It can be a challenge to speak to an individual right away if you have concerns about billing, questions for nurse's and activities. The director does not seem to be visibly involved in the day to day but I could be completely wrong about that. From my own experience communication has been a challenge with them. If there is an emergency they do call right away. There are spaces that look like they could use some attention but in the same right, seem rarely used. The dining room is always clean, but I feel the food lacks nutritional value, this can be because for many reasons, lack of staff, lack of space to prep, cook and serve, not enough experience kitchen staff and the menu is giving them from the main Brookdale head quarters/ Chef. The building is locked at specific time at night, providing safety both from within sides and out. I feel my experience with Brookdale Bath has been ok.

after the initial move in it dropped to possibly a 1. Cleanliness of the facility on a good day is a 2, .Friendliness - daytime - 4; evening/night 1.5. Administrative staff is not straight forward 1. What they could improve on -Communication with residents and families. Compassion for residents. Consideration for residents' belongings (Many times, soiled sheets were in my loved ones' closet, making everything stink. Then, when washed- items were washed with bleach that irritated my loved ones' skin and discolored all of his clothes) Work on communication tones - residents do not appreciate being talked to in condescending manners. The facility had nice qualities. But they are greatly over shadowed by the issues. With the 6mo that my loved one was there the entire staff outside of admin changed. Nursing director. Marketing person. Care staff. It was hard to keep up with who was taking care of the residents. There was some misleading things that occurred. My loved one should never have been accepted there. He needed a higher level of care, but they assured me they would take care of him. After multiple falls, and trips to the ER a hospital stay and rehab stay, it was no appropriate for him. But money talks.

I can only comment on dementia patient care. On weekends there were times nurses aids were pulled from the other departments and I don't think they were fully trained in caring for dementia patients. The dementia residents were confined to a single wing of the facility. The in/out door slams too loud and the small hallway where they kept the residents always had a TV blaring too loud. The staff were wonderful and it was a great place, but not the right place for dementia care.

Initially it is great, the honeymoon phase - they get you in the door and it is good. As your loved one is settling in it changes. Rooms are to be cleaned by staff at least once per week. It doesn't happen. Housekeeping is subpar all around the building. Overall it is not the worst place.
